New Patient Registration

There are two practices based within Tain Health Centre and both of these practices cover the same catchment area. It is each individual patient’s right to choose which Practice or General Practitioner they wish to register with. If you have been recommended a particular doctor please let the reception staff know so that they can guide you to the correct practice.

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. 

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Registration form

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Patients will remain registered with the practice and be entitled to treatment until either:

  • they choose to register with another practice or
  • the practice removes a patient from the list due to unacceptable behaviour as detailed in the “Health and Safety Policies”
